Create care plans

Bp Premier provides a range of Enhanced Primary Care tools (EPC) for managing patients with chronic disease. Care plans allow GPs to record related information and set goals and tasks in consultation with the patient to manage chronic diseases.

You can also create an EPC care plan template with default problems, goals, and tasks, and load the template into a patient record so providers don't have to create a new care plan for patients with similar chronic diseases.

Care plans are managed from the Enhanced Primary Care section of the patient record.

Create a care plan for a patient

  1. From the main screen of Bp Premier, press F2 or select FileOpen patient to select a patient and view the patient record.
  2. Expand the Enhanced Primary Care heading in the left hand tree and select Care plans.
  3. Click the Add button. The New Care Plan screen appears.
  4. Select to create a new GP Management Plan or Team Care Arrangements.
  5. Tick the Preload with data from previous care plan checkbox if a previous care plan exists and you want to carry over the details into the new plan. Click Ok.
  6. Either the GP Management Plan or Team Care Arrangements screen will be displayed, but the screens are very similar and the creation process is the same.

  7. Click Problem to open the Problem screen. To add a problem, you can:
    • Enter a problem as free text into the text field at the top.
    • Select a condition from the Problem list.
    • Select New condition and enter search text in the Search field to load new conditions into the Problem list.
  8. Add as many problems as required by the care plan. Select the problems you want to add and click Add.
  9. Click Goals to open the Goals screen. To add a goal, you can enter a goal as free text, or select from the Goal list.
  10. Select as many goals as required by the care plan and click Add.
  11. Click Task to open the Tasks screen.
  12. To add a task, enter the task as free text or select from the Task list.
  13. NOTE  Care plan goals and tasks can be customised and provided as lists, rather than entering free text.

  14. Select who is responsible for facilitating the task or tasks with the patient: the GP, practice nurse, patient, or external provider. If you select an external provider you must nominate a contact from the Select provider button.
  15. Select the Due date for the task.
  16. Click Save to save the task to the care plan.
  17. Add any Other comments about this care plan.
  18. Select the provider who created the care plan from the Performed by drop-down.
  19. Select a Date for review of care plan, to assess and adjust the care plan.
  20. Tick the Send reminder checkbox to generate a reminder for the patient to come back on the care plan review date.
  21. Click Save to save the care plan as a draft.

NOTE  Primary care templates like care plans and health assessments are not finalised until they have been printed and handed to the patient. After a care template is saved and printed, the plan cannot be edited.

Import a care plan

  1. From the patient record, expand the Enhanced Primary Care heading in the left hand tree and select Care plans.
  2. Click Add to open the the New Care Plan screen.
  3. Select a GP Management Plan or Team Care Arrangements, and click Ok.
  4. Click Import from the row of buttons along the bottom. The Import Care Plan screen will appear.
  5. Select the care plan you want to import data from and click Import. The previous care plan details will be loaded into the new plan.

Load a care plan template

  1. From the patient record, expand the Enhanced Primary Care heading in the left hand tree and select Care plans.
  2. Click Add to open the the New Care Plan screen.
  3. Select a GP Management Plan or Team Care Arrangements, and click Ok.
  4. Click Template from the row of buttons along the bottom. Select a template to load from the Care Plan templates screen and click Select.

You can edit the default template used when printing out a care plan. See System Templates for more information.

Add tasks and goals

Problems, goals, and tasks can be picked from lists or typed in as free text. Problems are selected from the conditions defined in Bp Premier. Goals and tasks are defined from the Configuration screen.

  1. Log in to Bp Premier as a user with access to configuration settings.
  2. From the main screen, select Setup > ConfigurationCare plans.
  3. Click Add next to the Care plan goals list to open the Care plan goals screen.
  4. Enter a new care plan goal and click Save to add the goal to the list.
  5. Repeat for Care plan tasks.
  6. Click Save to save the care plan lists. The new items will be available when a care plan is added from the patient record.

Finalise and print

  1. From the patient record, expand the Enhanced Primary Care heading in the left hand tree and select Care plans.
  2. Double-click the draft patient care plan you want to finalise.
  3. Click Preview to view a print preview of the care plan without printing.
  4. If you are satisfied the care plan is complete, tick Print patient copy to print a second copy to hand to the patient and click Print. The care plan will be sent to the printer and locked in Bp Premier. You will no longer be able to edit the care plan.

After a care plan has been finalised, if you click Add when Care plans is selected in the patient tree, Bp Premier will prompt if you would like to perform a care plan review. If you select Yes, a new care plan will open with the data from the previous plan. When the new plan is saved, the plan will be displayed in the Care plans list as a care plan review.
