Use Lyrebird for Consults and Dictation

This article describes using Lyrebird for consults and dictation in Bp Premier.

To use Lyrebird in Bp Premier, you must:

Consult is Lyrebird’s core feature. Lyrebird listens to your consultation with a patient in the background and generates complete clinical notes and any letters or documents you need in seconds.

TIP   Lyrebird allows you to customise the headings, structure, and content of your patient consultation notes to your preferences. For more information, see Customisation in Lyrebird.

  1. From the patient record, click the Lyrebird Health icon on the toolbar. The Lyrebird Health browser will appear.

  2. From the menu in the Lyrebird Health browser, click Consult. The Consult screen will appear.

  3. The Patient's name and Sex are automatically populated when Lyrebird is accessed directly from the patient record.
  4. Select your preferred Note type:
    • SOAP: follows the Subjective, Objective, Assessment and Plan structure.
    • Chief Complaint: includes multiple sections; chief complaint, systems review, medications, and planning.
    • Multiple Complaints: similar to chief complaint but will include more detail for secondary complaints.

  5. Optional: If you consult in languages other than English, Lyrebird can now listen in those languages. Choose your Language Settings from the available options.

    6. At the end of the consultation, Lyrebird will translate your clinical notes and letters into English.

    Lyrebird currently supports 10 languages and will expand to include more in the future. Visit Lyrebird Health Help Centre for more information.

  6. Hover over Start listening and click Initial Consult or Review Consult.

    Initial Consult

    Lyrebird will emphasise the history of any issues, particularly subjective information.

    TIP   Lyrebird Health recommends selecting Initial Consult when treating a new problem; this ensures that the most information is captured to get the best possible notes.

    Review Consult

    Lyrebird will concentrate less on the history and more on any changes in presentation from the previous consultation.

  7. Obtain the patient's Consent to generate clinical notes using Lyrebird's Consult Transcription service.

    8. Optional: Tick Telehealth Consultation. For Telehealth consultations using your computer, you can isolate the audio from any service or video conferencing platform to ensure that Lyrebird can hear both you and the patient, even when using a headset.

    To specify the patient's consent preference, select No or Yes, begin.

    EXAMPLE   

    Lyrebird has provided some example wording that can be used to obtain the patient's consent to use the Consult Transcription service to create patient notes.

      "Please confirm that you are comfortable with us taking an audio recording of your consultation to prepare a note of the consultation, which is then reviewed, finalised, and stored for inclusion in your medical record".

    Lyrebird will capture and record the patient's Consent in the transcribed clinical notes. When the transcribed clinical notes are saved from Lyrebird to Bp Premier, the patient's consent will be recorded in Today's notes.

    EXAMPLE   
    • Lyrebird will capture and record the patient's consent in the transcribed clinical notes.

    • When the transcribed clinical notes are saved from Lyrebird to Bp Premier, the patient's consent will be recorded in Today's notes.

  8. The Lyrebird Health browser may prompt you for permission to use your microphone; click Allow.

  9. For Telehealth Consults:
    1. An informational Telehealth Consult screen will appear. Click Continue.
    2. Select the Window where your Telehealth Consult is running, or select the Entire screen.
      3.
    3. Click Share.

      4. For more information about the Telehealth Consult feature offered by Lyrebird, visit the Lyrebird Health Help Centre.

  10. Lyrebird will Start listening to and transcribing the patient consultation.

    11. NOTE  Lyrebird Health destroys the original consultation audio files from their servers immediately following transcribing. Transcription occurs in real time, and any personally identifiable information is immediately redacted.

  11. Additional information about the patient can be freely typed in the Notepad textbox; for example, you could include information you do not want to verbalise to the patient. All free typed information will be recorded in the notes at the end of the consultation.
  12. To pause Lyrebird during the consultation, click Pause; Lyrebird will stop listening. When you are ready to continue, click Resume; Lyrebird will resume listening.

  13. When you have completed the consultation, click Stop. Lyrebird will immediately begin transcribing your notes in less than four seconds.
  14. If you need to Edit your notes, you should do so directly in the Lyrebird Health browser.

    15. TIP   Lyrebird learns from every edit you make in the browser and, over time, will generate notes that are personalised to your style and structure.

  15. To modify the note template after Lyrebird has generated the clinical notes, click on the + icon. The Note template screen will appear.

  16. Choose the Note template type from the available options. If required, specify any changes you wish Lyrebird Scribe to make, such as omitting the patient’s name, and then click Generate.

    17. Lyrebird will regenerate the clinical notes based on the Note template type you have selected. To view past and newly generated clinical notes, click on the relevant Note tab.

  17. AI edit allows you to make specific changes to your current note or document by using the transcript.

    18. Click AI Edit; a text box will appear, showcasing examples of options you can request, or type the changes you want the AI Edit to perform, and press Enter on the keyboard to submit your changes.

  18. To save your consult notes to Lyrebird, click Save note. Consult notes saved directly to Lyrebird can be accessed through the History tab and stored under the patient's name.

    19. TIP   If the Autosave consultations and documents preference is enabled in Lyrebird, the Save Note button will not be displayed. See Autosave consults and document for more information.

  19. Has the patient raised another concern after Lyrebird has finished transcribing the clinical notes? Click Resume Consult; Lyrebird will start listening and generate the clinical notes to incorporate the additional information.

  20. Would you like Lyrebird to generate consultation documents such as Medical Certificates, Patient and Specialist Letters or Mental Health Care Plans? For further details, see Generate Consult Documents; otherwise, proceed to the next step.
  21. To save your consult notes to Bp Premier, click Save to Bp. The Add and Edit Observations before sending to Bp screen will appear.

  22. If clinical Observations are detected during the consultation, Lyrebird will automatically add these to the Add and Edit Observations before Sending to Bp screen.

    23. IMPORTANT   Observations that are automatically populated must be reviewed for accuracy before saving to Bp Premier.

    If no clinical Observations are detected during the consultation, the clinician can manually enter these into the Add and Edit Observations before Sending to Bp screen.

    Observations will be saved as atomised values in Bp Premier; atomised values can be graphed to offer clinicians more insight into historical trends.

  23. Click Save to Bp. The Lyrebird Health browser will close.
  24. The clinical notes transcribed by Lyrebird will be inserted into Today's Notes section of the patient record.
    25. TIP   

    In Spectra, Clinical notes transcribed by Lyrebird Scribe will, by default, be placed at the Top of Today's Notes. If you would like the notes to be included at the Bottom of Today's Notes:

    1. Go to Setup > User Preferences > Clinical.
    2. Locate the preference Lyrebird transcription save to visit notes.
    3. Select the Bottom radio button, and then click Save or Save and Close.

    When clinical notes are saved from Lyrebird to Bp Premier, a statement will be included to confirm that the notes were generated via Lyrebird Health and have been reviewed by the clinician.

Manage Lyrebird browser sessions in Spectra and above

Enhancements have been made to Spectra to prevent multiple Lyrebird Scribe browsers from being opened simultaneously; only one Lyrebird Scribe browser can be opened at any given time. This change reduces the likelihood of recording multiple patient consultations in a single transcription or storing them in the incorrect patient record.

Manage multiple Lyrebird browser sessions in Orchid SP2

In Orchid SP2, Bp Premier allows you to open multiple patient records simultaneously, and from each patient record, you can launch an associated Lyrebird Health browser.

To bring the Lyrebird Health browser into the foreground for the associated patient, click the Lyrebird Health icon on the toolbar of that patient record.

EXAMPLE

To bring the Lyrebird Health browser to the foreground for patient Mrs Fran Deadly, click the Lyrebird Health icon from the toolbar.

What happens if I forget to pause or stop the previous Lyrebird consult?

Lyrebird does not pause or stop listening when a new Lyrebird consult is started from a separate Lyrebird Health browser; both Lyrebird Health browsers will be actively listening. As a result, the patient's consult notes may be generated incorrectly.

Before committing consult notes generated by Lyrebird to Bp Premier, the clinician must verify the accuracy and ensure that pertinent information has been included by reviewing and, if required, editing the consult notes.

What happens if I close the patient record but leave the Lyrebird browser open?

When closing a patient's record, the user will be prompted if the Lyrebird Scribe browser for the associated patient is still open. If you proceed to close the patient record, the Lyrebird Scribe session will also close, and you will be warned that you may lose any unsaved notes.

Click Yes, both the patient record and the associated embedded Lyrebird browser window will close.

Click No, the patient record will remain open, and the associated embedded Lyrebird browser window will be displayed.

With dictate, you can talk directly to Lyrebird, and Lyrebird will format what you say into letters and documents.

NOTE  Letters and documents generated using Lyrebird must be manually copied from the Lyrebird browser and pasted into a word processor document or template in Bp Premier.

  1. From the patient record, click the Lyrebird Health icon on the toolbar. The Lyrebird Health browser will appear.

  2. From the menu in the Lyrebird Health browser, click Dictate. The Dictate screen will appear.

  3. Optional: Enter the Patient's Name and Sex.
  4. Select Direct Dictate or hover over Smart Dictate and select a Dictation Type from the drop down menu. Types of dictation available for selection:

    Direct dictate transcribes your words verbatim with only minimal punctuation adjustments.

    Smart Dictate

    Smart dictate functions more like a conversational scribe, allowing you to dictate without worrying about misspellings or formatting.

    Medical Certificates

    Dictate into a document will format your dictation according to the selected template structure. This means that Lyrebird will slightly apply more editing on what you say compared to Direct Dictate.

  5. The Lyrebird Health browser may prompt you for permission to use your microphone; click Allow.

  6. Lyrebird will begin listening, you can commence dictating.

  7. If you need to pause Lyrebird during the dictation, click Pause, and Lyrebird will stop listening. When you are ready to continue with the dictation, click Resume, and Lyrebird will resume listening.

  8. Click Stop once your dictation is complete. Lyrebird will automatically start writing your letter or document.

  9. If you need to Edit your letter or document, this should be completed directly in the Lyrebird Health browser.

    10. TIP   Lyrebird learns from every edit you make in the browser and, over time, will generate notes that are personalised to your style and structure.

  10. To save the letter or document to Lyrebird, click Save document. To save documents directly to Lyrebird, the patient's name must be entered in the upper left corner of the Lyrebird Health browser.

    11. TIP   If the Autosave consultations and documents preference is enabled in Lyrebird, the Save Note button will not be displayed. See Autosave consults and document for more information.

  11. Click Copy dictation to copy the contents of the letter or document, which may then be pasted into a Word processor document or a template in Bp Premier.
  12. Once your dictation is complete, click the X at the top of the Lyrebird Health browser to close the window.

Lyrebird Health has an optional account setting that allows clinicians to Store audio recordings from dictations. This feature allows clinicians to conveniently playback and listen to recordings after completing the dictation. The account setting for Store audio recordings from dictations is turned off by default. To enable this feature, go to Enable Lyrebird Health to save audio recordings from dictations.

The audio recordings from dictation are stored in the Past Documents section on the History screen and are stored under the patient's name.

NOTE  Consult notes, documents and dictation recordings saved directly to Lyrebird are only retained for seven days by default before being permanently deleted; this period can be reduced to 24 hours or extended to six months. See When should Lyrebird Health delete saved items? for more information.

  1. From the menu in the Lyrebird Health browser, click History. The History screen will appear.

  2. To access documents created through dictation or playback the associated audio recording, search for the patient's name in Past Documents and click Open on the document you want to view. The document generated via dictation will be displayed.

    3. The audio recording will be displayed at the top of the screen, while the document generated from the dictation will appear below the audio.

  3. The audio playback feature allows the clinician to review the document's content for accuracy. It is recommended that any Edits that need to be made to the document be performed directly within the Lyrebird Health browser.

    4. TIP   Lyrebird learns from every edit you make in the browser and, over time, will generate notes that are personalised to your style and structure.

  4. To save the document to Lyrebird, click Save Changes.
  5. Click Copy notes to copy the contents of the document, which may then be pasted into a Word processor document or a template in Bp Premier.

Saved patient consult notes and documents such as letters and referrals can be viewed by clicking the History button in the Lyrebird Health browser.

  1. From the patient record, click the Lyrebird Health icon on the toolbar. The Lyrebird Health browser will appear.

  2. From the menu in the Lyrebird Health browser, click History. The History screen will appear.

  3. To view saved past consults or documents, search for the patient by name and click Open on the required consult or document.
  4. From the History > Past Consult screen you have the option to:
    • Update and copy patient notes.
    • Create patient letters and referrals to specialists.
    • Develop Mental health care plans.
    • Generate Medical certificates.

  5. From the History > Past Documents screen you have the option to:
    • Edit the document directly in the Lyrebird Health browser and save changes.

      • TIP   Lyrebird learns from every edit you make in the browser and, over time, will generate notes that are personalised to your style and structure.

    • Copy the notes and paste them directly into a word processor document or template in Bp Premier.

  1. Log in to your existing Lyrebird Health account or open a patient record in Bp Premier and click the Lyrebird Health icon on the toolbar.

  2. From the Lyrebird Health browser, click Support. The Lyrebird Support panel will appear.

  3. From the Lyrebird Support panel, you can send a message directly to Lyrebird Health Support, join the Lyrebird Community, download the Lyrebird Consent Support bundle, or visit the Lyrebird Health Help Centre.

This section provides some troubleshooting advice for common problems encountered using Lyrebird Health within Bp Premier.

Issue

Solution

To access Lyrebird within Bp Premier, your practice must have WebView2 enabled.

If WebView2 is disabled at your practice, clicking the Lyrebird Health button in the patient's clinical record will prompt the user to contact Bp Premier Support for assistance in activating WebView2.

After clicking block on the pop-up, users cannot modify their browser permissions for microphone access.

To modify the browser permissions to allow microphone access:

  1. Close Bp Premier.
  2. In the File Explorer, navigate to the following location:
    3. C:\Users\%userprofile%\AppData\Local\Best Practice\EBWebView\Default

  3. Locate and delete the Preferences file.
  4. When launching Lyrebird in Bp Premier, a pop-up will appear requesting microphone access; click Allow.
