ADF Post Discharge GP Health Assessment

The ADF Post Discharge GP Health Assessment is designed to assist with identifying and diagnosing the early onset of physical and mental health problems among former serving ADF members.

The Health Assessment is available to all former ADF members, including former serving members of permanent and reserve forces. It can be performed at any point after the patient’s discharge from the ADF. This health assessment may only be claimed once by an eligible patient.

  1. Open the current or former ADF patient for whom you want to conduct the assessment.
  2. From the patient tree on the left hand side, open Enhanced Primary Care and select ADF Post Discharge GP Health Assessment.
  3. Click Add. The ADF Post Discharge GP Health Assessment screen will appear.
  4. Work through the tabs down the left hand side, completing all sections. If you need assistance completing the form, click Quick Reference Guide or Guidelines for GPs to open the document viewer with help.
  5. Click Preview to view a print preview of the finished form. Click Save to save the record, or Print to print to the workstation printer.
  6. If you need to update the form later, click View to re-open the form.
