Bp Premier provides several Enhanced Primary Care tools within the patient record to manage chronic illnesses. These include:
- EPC Care Plans
- Dementia assessment tool
- Diabetes cycle of care
- Age-related health assessments
- Medication review
- ADF Post Discharge GP Health Assessment
Word processor templates are also available in Bp Premier for managing chronic illnesses. Some users may prefer to use a word processor template for Enhanced Primary Care as information can be added to word processor templates more freely compared to Enhanced Primary Care tools. However information added to a word processor template does not feed back into the patient clinical record as it would if added via Enhanced Primary Care tools.
