The Close the Gap (CTG) PBS Co-payment Measure assists eligible Aboriginal Australian patients by making PBS medications more accessible through a co-payment relief system. The Close the Gap initiative commenced on 1 July 2010.
Before you can apply CTG to a patient in Bp Premier:
- A practice must complete and submit the PIP Indigenous Health Incentive practice register authority form (IP009), available on the Department of Human Services website.
- Patients identifying as indigenous who attend a registered practice with chronic disease or risk factors can have their practitioner complete the PIP Indigenous Health Incentive patient registration and consent form (IP017), available on the Department of Human Services website. Patient consent must be completed and retained at the practice.
You can search for the above forms, and all other PIP and CTG forms, at the Department of Human Services form search page.
The CTG Registration and Consent and CTG Patient withdraw consent forms are also available as templates in the word processor.
To make a patient eligible for the CTG PBS copayment measure, tick the Registered for CTG PBS co-payment relief checkbox in the patient' s demographic details, as shown.
